Services: Hospice of Rockingham County is the only full-service nonprofit hospice located in Rockingham County. In addition to inpatient care in our unique 8-bed residential facility, hospice services are provided in the home or wherever home may be – including SNFs, ALFs, private care homes, and group homes. Hospice of Rockingham County primarily serves Rockingham County residents however, we do serve patients located along the county lines in Caswell, Guilford, and Stokes counties. Hospice services include in-patient/residential facility; 24-hour on-call availability; after-hours/weekend admissions; hospice nurse case manager (including coordination/management of all DME, pharmacy needs, & supplies related to terminal dx); medical social work; spiritual support; in-home hospice aide services; volunteer services; 13-month bereavement follow-up. |
